Ummah Welfare Trust is grateful to you for your consent to claim the gift aid on your kind donation. This means that the charity will receive an extra 25p from HMRC for every pound you donate. In agreeing to Ummah Welfare Trust to claim the gift aid, you confirm that:
1. You are a UK taxpayer.

2. You would like Ummah Welfare Trust to treat all donations you have made for the past four years and all future donations as Gift Aid until you notify otherwise.

3. You confirm that you are paying an amount of income tax and/or Capital Gains Tax for each tax year, that is at least equal to the tax all charities and Community Amateur Sports Clubs (CASCs) will reclaim on your donations in the appropriate tax year.

4. You understand that if you pay less income and/or Capital gains tax to cover the amount of Gift Aid claimed on all your donations in that tax year, it is your responsibility to pay any difference.

5. You agree Gift Aid will be used for funding administration of the charity as well as charitable projects.

6. You will notify Ummah Welfare Trust of any change in your personal circumstances, such as name and address or that you no longer pay UK Income or Capital Gains Tax, or you wish to cancel the declaration.
